$75 per player by Friday, October 9. $85 per player on or after Saturday, October 10th (covers green fees, cart, lunch, polo shirt, goody bag, & awards ceremony meal)

Pre-registration: Register by Friday, October 9th-$75 per player/$ 300 per team. Register on or after Saturday, October 10th—$85 per player/$340 per team. The team captain should fax or mail the completed form for the entire team to the Foundation at the fax #/address below. Full payment for the entire team should be made by check or credit card when registering. Reservations at the course will be confirmed once registrations are paid in full. Sorry, no refunds after Friday, October 9, 2014.
On-Site Check-In: The team captain and all team members should check-in at the same time to receive your starting hole assignments.
