Disaster Relief – $477,376 to 1,470 Officers and Staff

Use this form to cancel your membership if you do not want your membership contribution to increase to $2 effective March 16, 2018.

We will process your cancellation and you will no longer be eligible for full financial assistance should an emergency occur.
